Council on Aging's Adult
Day Care provides a therapeutic environment
for St. Lucie County adults.
Who
is Eligible for Adult Day Care?
Adult Day Care clients
include adult residents of St. Lucie County
over the age of 59* who may be socially
isolated, physically impaired, limited
in their ability to function independently
in the community, or may have some degree
of mental confusion and require supervision.
*Exceptions
to the age criteria may be made at the
discretion of the President/CEO.
How
Do I Apply?
Contact Council on Aging
Information and Referral at 465-1485 (Fort
Pierce) or 336-8608 (Port St. Lucie).
A brief telephone intake will be completed.
A case manager will then phone you to
schedule a visit to one of our Day Care
Centers.
After touring with your loved one, a comprehensive
assessment will be conducted to determine
your abilities and your needs. We will
give you a medical form that must be completed
by your physician.
When you have your completed medical form,
contact the case manager to arrange your
schedule of attendance.
